Lease Agreement
This agreement, made and executed this ______ day of ____________, ________ between ____________________
owner/manager, hereinafter called 'lessor' and ________________
hereinafter called 'lessee', withnesseth that:

1. Description of Premises: lessor hereby leases unit # _____located at ________________________ to be used and occupied by th elessee as a residence and for no other purpose whatsoever.

2. Term of lease: This lease is at least for one year, beginning ________________and ending ____________________.

3. Rents and charges: Lessee agrees to pay to lessor as rent the sum of $___________ divided into twelve equal payments of $______ payable monthly in advance of the first day of each month. Rent not received by midnight of the third day of any month shall be considered late and will be charged an additional $10 per day. Late rent with correct late fees not paid by the tenth day of the month will result in the issuance of an eviction notice. A $25 service charge will be assessed against lessee for any check returned due to insufficient funds, or for any other reason, and a penalty of $10 per day from the 3rd of the month. Occupancy for one or more days of any calendar month makes lessee liable for that entire months rent.

4. Security deposit and damages: Lessee agrees to deposit upon the execution of this lease, in addition to the first months rent, the sum of $________, which sum shall be security for the strict performance of all lessee's obligations under this lease and shall also serve as a fund from which the lessor shall be reimbursed to compensate for unreasonable wear and tear on the part of the lessee, failure to clean the apartment after vacating, missing articles, such as keys, garage remote transmitter, curtain rods, ect. The security deposit is NOT to be deducted from the last months rent, NOR is it to be used in lieu of the last months rent. According to Ohio law, no interest is due on any security deposit equal to or less than one months rent.

5. Breaking the lease: If lessee moves before the lease term has expired, the lessee shall be liable for all damages(monies lost) by lessor by reason of lessee move. The amount due lessor shall include but not be limited to liquidating damage equal in amount to two months rent plus forfeiture of the security deposit.

6. Condition of premise and inspection: The premises are now in good repair, normal wear and tear expected. Lessee shall give lessor prior notice of any defects or breakage in the structure, equipment, or fixtures on the premises. Lessor may at all reasonable times enter the premises to inspect and care for the same, or to make repairs. (General rule is 24 hour notice, unless of emergency). NO painting to be done by the lessee.

7. Rules and Regulations: Lessee agrees to comply with (1) all rules and regulations of the comples and these shall become binding upon signing this lease. (2) All laws of the state, (3) All ordinances of this municipality. Lessor reserves the right to make such rules and regulations that in its judgement may from time to time, be needed and proper and upon delivery of the changing rules and regulations to the lessee either in person or by mail, they shall become binding.

8. Automatic Renewal: In the event the lessee shall fail to vacate and surrender the leased premises on or before the termination date of this leaseas hereinbefore provided, or to deliver to lessor in person or by mail at least 30(thirty) days before the expiration of this lease as hereinbefore provided written notice, stating his intention not to renew the term of the lease, pursuant to the option herein granted, this lease shall , at lessor's option be deemed renewed for an additional on the same terms, covenants, and conditions. It shall continue to be at lessors option, automatically renewed on expiration of each renewal term on the failure of lessee to vacate and surrender the leased premises on or before the expiration of said renewal term or give the notice required of his election not to renew the term of the lease, provided however, and that this lease shall terminate in any event on or before _______________________________.

9. Occupant's liability-Lessee agrees that NO ADULT who does not sign this lease will occupy the premises and that no minors other than _______________________________(ages)_______________
will be housed in the premises unless an additional child be born or adopted by lessee during tenancy. Non related adults sharing an apartment are each liable for the entire months rent, not just their prorated share thereof, and payment is to be made one check per apartment.

10. Assignment: This lease shall NOT be assigned or sublet. This lease is NON TRANSFERABLE.

11. Disturbance of the Peace: if the lessor, at any time, shall deem the occupants of the leased premises or their conduct objectionable, and if the lessor shall give the lessee three(3) days notice of lessors intent to terminate the lease, then this lease shall, at the expireation of such period of three days, terminate.

12. Lessor Non-Liability: Lessor shall not be liable for any damage to any person or thing, however occuring by or from gas, electricity, fire, water, ice, snow, or sewerage, nor shall the lessor be responsible for any damages to lessee, or any occupant of the dwelling, resulting from any other cause whatsoever unless willful negligence of the lessor. Lessor is specifically NOT responsible for any loss of damage to lessee's automobile(s) or contents, nor for damage to lessee's clothing while using laundry facilities, not for belongings stolen from the apartment or storage locker. Lessor is not responsible for food spoilage due to refridgerator malfunction or electric failure.

13. Lessee is responsible for maintaining insurance to cover fire, theft, insurability of his/her apartment.

15. NO PETS ARE ALLOWED unless specifically approved by building owners/managers. Any tenant with an unauthorized pet WILL RECEIVE AN EVICTION NOTICE, given three(3) days to vacate the premises. Pet Approval? (yes) (NO)
